Splenomegaly is mostly found when the Hb is low, either because the patient has thalassaemia intermedia or because of thalassaemia major, when the pre-transfusion Hb is kept low (below 9g/dl). Low Hb is of course one of the causes delayed growth. Also splenomegaly can trap a lot of blood cells and cause a condition called hypersplenism (with increased needs for blood transfusion, low white cell and platelets)this also is accompanied by poor growth. If the patient receives blood when Hb is over 9g/dl from early childhood not only will growth be better but spleen will not enlarge and the need for removing it will not arise.
